We have received a referral letter from your dentist requesting an Orthodontic Consultation.
You are now registered with our practice, and we would be delighted if you could go to https://orthowise.com/mh_pds/
You will find there a COVID-19 Orthodontic Treatment Consent Form and a Confidential Medical Form that are required to be completed before scheduling your appointment with us. Upon submitting both forms you will be taken to the online booking page where you will be able to book your appointment. The whole process needs to be completed by a Parent or Guardian if the patient is under 16 years of age.
The practice is open only to Patients with pre-arranged appointments. Parents/Guardians/Chaperones will be asked to remain outside where safe to do so, whilst observing social distancing measures.
All patients must attend wearing a face mask/face covering.

Late arrivals will not be seen and will need to be rebooked by contacting our Reception Team. Additionally, we would appreciate it if you update any contact information we hold.
Children under 18 years of age are required to be accompanied by a Parent or an Adult Guardian for the initial assessment appointment only. If you had any dental x-rays (panoramic OPT, Cephalometric X-ray) taken recently, please bring or send them to us prior to your appointment.
Free orthodontic treatment is sometimes available on the NHS, although you can also pay for private treatment. A rating system known as the Index of Orthodontic Treatment Need (IOTN) is used to assess your eligibility for NHS treatment. If you or your child don’t qualify for NHS orthodontic treatment, or you don’t want to wait for NHS treatment, you can pay for private treatment, an estimate of the treatment cost will be provided after an initial assessment.
