We have tried to contact you on several occasions via email, text message and postal letter. This is our last attempt to schedule an appointment before returning the patient care back to the referring dentist. If a recall appointment is not booked within the next 7 days. Any further appointments will require referral from the dentist or be performed on private patient basis.
To schedule the recall appointment, we would require for you to complete a COVID-19 Orthodontic consent form that can be accessed via https://orthowise.com/patient-portal/.
Once the Consent Form has been fully completed you will be automatically redirected to our booking system where you will be asked to:
- Choose from the dates and times available and then press Continue.
- On the next page, you will be asked to enter Patient’s full name, contact email address and telephone number, and also Patient number (Patient number can be found on top of this email).
- When you have entered all the required details, please press Confirm.
Our Reception Team will confirm the booking within 24 hours by email provided in the booking form.
Please note: The practice is open only to Patients with pre-arranged appointments. Parents/Guardians/Chaperones will be asked to remain outside where safe to do so, whilst observing social distancing measures.
All patients must attend wearing a face mask/face covering.

Late arrivals will not be seen and will need to be rebooked by contacting our Reception Team. Additionally, we would appreciate if you update any contact information we hold.
